Reasonable Accommodation/Modification Information Sheet

We are committed to complying with the Fair Housing Act and Section 504 of the Rehabilitation Act by ensuring that its policies and practices do not deny individuals with disabilities the opportunity to participate in, or benefit from, nor otherwise discriminate against individuals with disabilities in connection with the operation of housing services or programs solely on the basis of such disabilities.

A request for Reasonable Accommodation/Modification can be made by any applicant, resident or other person on behalf of an applicant or resident by contacting your management team.

All applicants or residents will be provided with a request form when requesting a reasonable accommodation or modification. At the applicants/residents request, the Request Form may be completed by the applicant/resident, the applicant/resident’s designee or a member of the management team.

Applicants/residents or their designee may inquire at the office, at any time, for any information about requests.

We will reply to requests as quickly as possible, but no later than five (5) days after receiving a request to explain the status of the request and generally keep the applicant/resident and or their designee up to date throughout the request process.

We will approve or deny requests as quickly as possible, usually within thirty (30) days unless there is a delay in receiving any necessary verification from third parties. Requests with an obvious need will be approved within five (5) days of receiving the request. All decisions to approve or deny reasonable accommodation/modification will be communicated in writing, or if required, an alternative format.

Any applicant/resident that is denied approval of a request will have the opportunity, within fourteen (14) days of the denial, to submit an appeal to be reviewed by the next level supervisor.
